Use a brief decision record: date, question, chosen option, owner, reasoning, important trade-off and when it should be revisited. Link the supporting discussion or recording internally, but make the decision understandable without opening it. Mark superseded decisions so search results do not present an old answer as current.

Atlassian’s asynchronous collaboration guidance describes decisions and updates staying connected to the work they are about, embedded in Confluence pages and linked to Jira projects. The tools matter less than the rule: one authoritative record, clear ownership and a way to find it by topic or project.

When a live meeting settles an issue, assign someone to publish the decision before the next handover. If an uninvolved teammate cannot find the record in a short search, improve its title or location. Avoid a separate log that nobody updates. Embed the record in the workflow where the next decision-maker will look.
